All necessary documents for participating in the Olympiad are listed below. Document templates are available at the bottom of this page.

Document Submission Instructions

1. Prepare your documents:

• Complete all required forms
• Scan each document clearly

2. Upload your documents:

• Team leaders must submit documents of each participant through the registration form

3. Bring originals to the event:

• All participants must present original documents at the Olympiad venue for verification.

For Participants:

1. Passport.
2. Application for enrolment (Template 1).
3. Consent of the legal representative acknowledging the Center’s terms and conditions (Template 2).
4. Consent to personal data processing (Template 3).
5. Consent to processing personal data processing for distribution (Template 4).
6. Notarized authorization (consent form) assigning responsibility for the underaged student’s health and safety to the team leader (Template 10).
7. Medical clearance certificate for sports activities (Template 8).
8. Informed voluntary consent for medical treatment (Template 5).
9. Consent of the legal representative (parent) for medical treatment (required for children under 15) (Template 6).
10.Certificate of absence of contact with patients with infectious within the last 21 calendar days (Template 9).
11.Test results for helminth eggs and enterobiasis (pinworm) screening (required for pool access).
12.Physician’s tuberculosis screening report (valid for no more than 1 year).


For team leaders and observers:

1. Passport.
2. Consent for personal data processing (Template 7).
3. Certificate of absence of contact with patients with infectious within the last 21 calendar days (Template 9).
