  • Q:
    Regarding a child’s name, I notice the form requests the order: Surname, First Name, and Patronymic. For consistency with their official documents, would it be acceptable to write the name exactly as it appears in the passport instead?
    A:
    Certainly. Please use the name order exactly as it appears in the passport.
  • Q:
    Regarding Form No. 5, specifically the 'Healthcare Worker' section: who is responsible for completing it? Is it intended to be filled out by our personal doctor, or is another party designated for this?
    A:
    For Form No. 5 the 'Healthcare Worker' section should be left empty. Our doctor will complete this section at the venue.

    Could you please confirm if a signature from a family doctor is sufficient to complete Forms 8 and 9, or if these forms require a signature from a specific type of medical specialist or authority?
    A:
    For Forms No. 8 and No. 9, a signature from a family doctor is acceptable.

    I am writing to clarify the timeframe for the medical certificate required for Form 9. The form states the individual must not have been exposed to an infectious disease within the past 21 calendar days. For our submission, does this 21-day period refer to the 21 days leading up to and including November 23rd?
    A:
    The ‘past 21 calendar days' means the 21-day period immediately preceding the date of submission or a key deadline. Therefore, if the required date is November 23, the 21-day period would be from November 2 to November 22, inclusive. The certificate dated on or after November 23rd would then attest to no exposure during that specific window.

    Could you please clarify the language requirement for the Form 11 and 12. Is it acceptable to submit these documents in English, provided they bear the official stamp and signature from the issuing medical facility, or are official Russian translations mandatory?
    A:
    All documents provided in English are perfectly acceptable. There is no requirement for official Russian translations.

    Could you please confirm if we are permitted to upload only the signature pages of the required forms, rather than scanning entire multi-page documents?
    A:
    Scanned copies of pages with handwritten text and signatures are sufficient. Please ensure you bring the complete set of required documents to the venue.

    I am seeking clarification on the medical documentation requirements. Is the Test Results for Helminth Eggs and Enterobiasis (Pinworm) Screening a mandatory submission for all students, or are there specific circumstances that would require it?
    A:
    During the Olympiad, students will have the opportunity to visit the swimming pool in their free time. To comply with the regulations, any individual accessing a public pool must provide documentation of a negative Test Result for Helminth Eggs and Enterobiasis (Pinworm) Screening. A clear, scanned copy of the results, which explicitly states that the tests for helminths and pinworms are negative, will be sufficient. Please note that this document is only required for students who wish to use the pool and is not mandatory for general participation in the Olympiad.

    I am writing to clarify the requirements for the Physician’s Tuberculosis (TB) Screening Report. Could you please explain what this report entails? Furthermore, is a record of the TB vaccination sufficient to meet this requirement, or does it require a more recent specific test, like a chest X-ray?
    A:
    Physician’s Tuberculosis (TB) Screening Report is mandated by health regulations due to the large number of participants residing at the Educational Center. Acceptable documentation may include a physician’s statement confirming the absence of TB symptoms, results from a negative TB screening test, or a valid TB vaccination certificate.

    We have a comprehensive, notarized document signed by the parents that addresses several requirements on the list, including parental and medical activity permissions. Would it be acceptable to submit this single document to fulfill these combined requirements, or are separate, specific forms mandatory?
    A:
    Please be advised that the Parental Consent and Medical Clearance forms are separate documents with distinct purposes and cannot be combined. The Medical Clearance form must be submitted directly to the venue’s medical office, while the Parental Consent form is required to be in your possession at all times.

    We are in the process of completing Form 10. Is it mandatory for this form to be stamped by a notary office, or is it sufficient to have only the parent’s signature? We ask because local notary offices typically only provide certification for documents in our native language.
    A:
    The signature of a parent or guardian is sufficient; notarization is not necessary. Signatures from one or both parents are acceptable.

    Could you please confirm if the upcoming Olympiad will be live-streamed for remote viewing?
    A:
    Yes, the Opening and Closing Ceremonies will be live-streamed. The streaming platform will be announced soon.
